Aim: improve individuals understanding about medication that post transplant patients are prescribed. Empower AHP to help patients become confident in self-administration of medication.

End goal:

To facilitate teaching patients about their medications with the goals to:

• Learn what / when/ what dose/ why are they taking these
• Practice self-administration to ensure confidence prior to discharge
• Learn how to prepare specific medications: mixing nebulisers, self administration of intravenous medications.
• Practice using patient diary (so called ‘blue book’) to trace compliance and understanding of their medications
